Error while installing ESXi 6.0 in HP BL460c G8 Blade.

Error: Input/Output error during read on /dev/disks/mpx.vmhba33:C0:T0:L0 WriteNewPtable: Unable to commit to disk

As observed, the Esxi hypervisor encountered an error while installing on the Physical Server.

Error: Input/Output error during read on

/dev/disks/mpx.vmhba33:C0:T0:L0

WriteNewPtable: Unable to commit to disk

Troubleshooting:-

From the vmkernel.log we saw the following error while the hypervisor tried to create a partition for Esxi on the local disk

Warning: Partition: 1157 : Partition table read from device mpx.vmhba33: C0: T0: L0 failed: I/O error.

Hence from the logs it was evident that there are either of these issues:

1: Issue with the ISO which is being used for installation.

2. ISO when trying to create a vFat partition is unable to read/write to local storage.

We tried the same ISO on another Esxi host and was able to successfully upgrade the Esxi host.

Henceforth, for this Esxi host, The ISO is not able to read/write to create the partition table on the disk.

Solution:-

To Fix, this issue, contact Hardware Vendor to check Disk attached to servers where you are installing Hypervisor. Should not have I/O error or any hardware related issue.
